Our person-centred care service enables you to work towards the goal you wish to achieve. To do this, we tailor what we do to suit your specific needs. We take our time to listen and understand you. We learn about what’s important to you, recognize your needs, and respect your right to make health decisions and choices. Furthermore, we also respect your preferences and diversity.
Most times, we work closely with your family, as well as other support professionals, to create the best person-centred plan for you in the form of a document. This means that you and your family get a say in the decision-making process when planning for your care and treatment. So you get to tell the story of how you want to live your life.
Our person-centred care plan includes stating what your goals are and how we will work together to help you achieve these goals. As time goes on, this document can be updated as your goals change. It is also made available to everyone involved, so we can refer to it whenever to make sure we are on the same page and that we are doing absolutely what’s best for you. We use different tools to communicate, so even if you have difficulty communicating verbally or in writing, you can still be in charge of developing your plan.
What is in our person-centred plan?
Our person-centred plan contains how you want to live your life, now and later. It also contains in detail the kind of support you need and what you intend to achieve with that support. Our workers work hand in hand with you to make this happen.
An example of what the plan contains:
- What is important to the individual
- What the individual wants to achieve
- Where the individual wants to live
- What the individual wants to learn
- What support the individual needs and how they intend to use it
- What the individual’s hobbies are
In summary, our person-centred service states out what is important to you as an individual, what you want to achieve, where you want to live, what you want to learn, the type of support you need and how you want to use it, how you can live a healthy life, your hobbies and a lot of other things that are specific to you. It’s ‘person centred’ because it is tailored to meet your specific needs. If your needs change, we make adjustments as well.
